Pathophysiology When the brain perceives stress, physiologic and behavioral responses are initiated leading to allostasis and adaptation. Over time, allostatic load can accumulate, and the overexposure to neural, endocrine, and immune stress mediators can have adverse effects on various organ systems, leading to disease. In simpler words, the body’s autonomic nervous system controls the heart rate, breathing, vision changes and more. Its built-in stress response, the “fight-or-flight response,” helps the body face stressful situations. When a person has long-term (chronic) stress, continued activation of the stress response causes wear and tear on the body. Physical, emotional and behavioural symptoms develop. Diagnosis Stress is subjective — not measurable with tests. Patient is the only one who knows the severity of his suffering. Some values like blood pressure can be on the higher side in some cases. It is actually a complication of herpes infection. A few of the infected people are affected with this complication. As it is a severe and painful condition, most of the cases are managed as emergency. It is an infection of a nerve and the skin around the area of nerve course. It is characterised by pain or a tingling sensation, in a limited area on the side of the face or torso, followed by a red rash with blisters. The fluid filled blisters spread fast and are extremely painful. SIGNS AND SYMPTOMS Unilateral, fluid-filled blisters with severe pain and burning sensation. Sometimes itching is present. The rash usually follows the course of the affected nerve. CAUSES It occurs over the dermatome innervated by the ganglion, where the virus(Varicella zoster) resides for life until stress, immunosuppression, or some other condition allows it to travel to the skin served by the affected nerve. PATHOPHYSIOLOGY Varicella-zoster virus is a highly contagious DNA virus. Varicella represents the primary infection in the nonimmune or incompletely immune person. During the primary infection, the virus gains entry into the sensory dorsal root ganglia. How the virus enters the sensory dorsal root ganglia and whether it resides in neurons or supporting cells are not completely understood. The varicella-zoster virus genome has been identified in the trigeminal ganglia of nearly all seropositive patients. The virus remains latent for decades because of varicella-zoster virus–specific cell-mediated immunity acquired during the primary infection, as well as endogenous and exogenous boosting of the immune system periodically throughout life. Reactivation of the virus occurs following a decrease in virus-specific cell-mediated immunity. The reactivated virus travels down the sensory nerve and is the cause for the dermatomal distribution of pain and skin lesions. DIAGNOSIS Clinical evaluation on the basis of the characteristic appearance of the rash. TREATMENTS A live, attenuated vaccine aimed at boosting immunity to Varicella zoster virus and reducing the risk of Herpes Zoster. The vaccine has been shown to reduce significantly the incidence of both HZ and Post Herpetic Neuralgia. PROGNOSIS Early recognition and treatment can reduce acute symptoms and may also reduce Postherpetic Neuralgia. Pathophysiology Degeneration of the rotator cuff tendon results from a variety of intrinsic factors including but not limited to age-related degeneration, inflammation, vascular changes, and oxidative stress. Among these elements, the key factor leading to rotator cuff weakness and degeneration is aging. Rotator cuff injury starts from trauma. Macro-trauma causes an acute tear, which is seen generally a younger patient resulting in a complete tear. Micro-trauma causes tendon degeneration and with insufficient healing, leads to degenerative tears. Typically, acute tears happen in younger patients, and degenerative tears occur in older patients. However, a smaller amount of force is needed to cause a complete tear if there is sufficient tendon degeneration. Diagnosis

The study was conducted to determine the relationship between rotator cuff (RC) tear and the orientation of the glenoid. Ninety‐six shoulders (94 patients) that underwent open RC repair were grouped according to the type of tear. They measured on MRI the acromio–glenoid angle (AG) and the supraspinatus fossa glenoid angle on the anterior–posterior (SGAP) and axial (SGAX) views. RC patients had a smaller AG angle and a larger SGAP angle compared to controls. It was also found a highly significant difference in glenoid version measured by SGAX between anterior cuff tears and posterior cuff tears. Furthermore, an association between RC tear and the orientation of the glenoid relative to the axis of the supraspinatus fossa was identified. Pathophysiology The process of allergen sensitization involves the participation of antigen-presenting cells, T lymphocytes, and B lymphocytes and depends on environmental allergen exposure. Sensitization results in the generation of allergen-specific IgE that circulates in the peripheral blood and attaches itself on the surface of all mast cells and basophils including those that home to the nasal mucosa. Subsequent nasal exposure to allergen activates these cells and, through the release of the classic mediators of the allergic reaction, produces acute nasal symptoms. Over a short period of time, these symptoms become indolent, whereas inflammatory and immune cell infiltrate, including eosinophils, basophils, neutrophils, T lymphocytes, and monocytes, characterizes the late stages of the allergic response. In parallel, and probably as a result of the development of mucosal inflammation, the nose becomes primed to allergen and reacts more vigorously to subsequent allergen exposure but also becomes hyperresponsive to irritants and to changes in atmospheric conditions.

A randomized, double-blind, placebo-controlled trial was done to assess the efficacy and safety of mepolizumab versus placebo for severe bilateral nasal polyposis. Patients aged 18 to 70 years with recurrent nasal polyposis requiring surgery were chosen. Patients received 750 mg of intravenous mepolizumab or placebo every 4 weeks for a total of 6 doses in addition to daily topical corticosteroid treatment. A significantly greater proportion of patients in the mepolizumab group compared with the placebo group no longer required surgery at Week 25. There was a significant improvement in nasal polyposis severity VAS score, endoscopic nasal polyp score, all individual VAS symptom scores, and Sino-Nasal Outcome Test patient-reported outcome score in the mepolizumab compared with placebo groups. In patients with recurrent nasal polyposis receiving topical corticosteroids who required surgery, mepolizumab treatment led to a greater reduction in the need for surgery and a greater improvement in symptoms than placebo.
